DiabetesAmerica and Curant Health Partner to Improve Outcomes for Diabetes Patients

November 16, 2016 - Atlanta & Houston - DiabetesAmerica, a provider of healthcare services focused solely on diabetics, and Curant Health, a leading medication management company, announced today the launch of a pilot program intended to improve outcomes for diabetes patients.

The short-term goals of the pilot are two-fold and incorporate Curant Health’s Patient Support Services and Healthcare Provider Communication Services programs. The pilot aims to improve the patient journey and to reduce the administrative challenges. Pilot locations include DiabetesAmerica centers in Arlington, Plano and Las Colinas, Texas.

Long-term partnership goals include sustained improvement in three of the following “Super Six” metrics: increasing the percentage of patients at A1C level below 7, increasing the percentage of patients at LDL cholesterol below 100 and increasing the percentage of patients at blood pressure readings of less than 130/80.

“Ultimately, our goal is to make a positive, lasting impact on the lives of our shared diabetes patients through our proven clinical model and by accessing Curant Health’s supplemental medication management, patient support services and aligned patient goals,” said Mike Puls, President and CEO, DiabetesAmerica.

Curant’s patient support services within the pilot include medication fulfillment, prior authorization assistance, home delivery, medication synchronization, assignment of dedicated patient care coordinators, medication therapy management, 24/7 access to pharmacists and medication shipment tracking.

“We invest in enabling diabetics to thrive under the conditions of their illness as members of patients’ extended care teams,” said Curant Health COO and Team Type 1 Board Member Marc O’Connor.

A 2013 study conducted by Curant Health pharmacists showed a 35% gain in adherence for Curant Health patients compared to patients receiving medications from retail pharmacies for patients with diabetes in Georgia.
