August 10, 2016 – Atlanta, Georgia – Curant Health, provider of personalized medication management services, has formed a partnership with Louisiana Physicians ACO (LPACO), to deliver a coordinated, structured medication management program for selected Medicare patients

Initial program focus will be on patients with one or more of the following chronic conditions: diabetes, congestive heart failure, chronic obstructive pulmonary disease (COPD), peripheral artery disease (PAD) or end stage renal disease (ESRD). Program participants will receive medication, dedicated care team support and 24/7 access to pharmacists. Eligible patients will also receive enhanced medication therapy management (MTM).
